CARE & SUPPORT SERVICES UNIT

Mission:To offer succor through the provision of comprehensive and quality palliative care services to all Persons living with HIV (PLHIV), TB patients, orphans and vulnerable children and persons affected by HIV and AIDS at IHVN supported facilities.

IHVN care and support services are provided at both facility and community levels to address:

  • Prevention of HIV transmission/ secondary infection (PHDP)
  • Psychological/Spiritual care
  • Socio-economic support/Income Generating Activities (IGA)
  • Food and Nutrition Support
  • Human rights and legal needs
  • Other wrap-around care

Care and Support Implementation Strategies

  • Capacity building for facility/NGO/FBO/CBO staff providing services to clients
  • Ensuring provision of user-friendly services (counseling, Health Education classes etc.)
  • Ensuring efficient clinic flow for clients through Nursing triage
  • Developing and deploying of services data collection tools
  • Forecasting & requisition of C&S commodities
  • Distributing of C&S commodities to Regions/sites
  • Establishing referral/linkages to other outlets for wrap around services
  • Initiating and creating support groups
  • Conducting advocacy visits to community stakeholders to create awareness about the program and request for selection of volunteers
  • Setting up CSS/CHBC service unit at facility level that will link services to the community.
  • Developing training curricula for different cadre of care providers
  • Identifying, selecting and training PLHIV to serve as peer educators
  • Conducting supportive supervision and mentoring/backstopping visits that provide on the job coaching to ensure quality service provision
  • Developing Information Education and Communication (IEC) materials to address information(knowledge), behavior and attitudinal gaps for clients & service providers
  • Ensuring the utilization & practice of universal precaution and especially injection safety by providers

Clinical Care and Support Activities at Facilities

They include:

  • Counseling/education on HIV basics, diagnosis, disease outcome, positive living, prevention of transmission/re-infection, malaria, safe water system etc.
  • Assessment of disease status/staging of clients
  • Checking for vital signs to monitor patients physiological progress, spot problems early and refer appropriately
  • Patient triage to identify clients requiring emergency or urgent attention
  • Referral to: - Support groups - Adherence counseling - Community home based care - Wrap around services at community level and other implementing partners and NGO outlets
  • Provision of a basic care package items which include, insecticide treated net, jerry cans and bucket, water safe and condoms, educational materials, antiseptic soap and calamine lotion. These items are an aid for self-care/education by clients themselves
  • Ensuring laboratory monitoring including CD4 counts, heamogram, clinical chemistry, pregnancy testing if indicated, malaria smears etc.

Adherence Counseling Services: The success of antiretroviral therapy lies in clients' ability to adhere to their drugs and clinic appointment. The Care & Support team works with the sites to identify adherence counsellors, volunteers and Treatment Support Specialist (TSS) for the provision of this service. Clients are counseled using adherence job-aids to achieve effective treatment preparation, education and adherence.

Nutrition Services– Providers are trained to conduct nutrition assessment for all clients. Malnourished clients receive ACTION meal, a ready to use therapeutic food made from maize, soya beans, groundnuts which are all readily available food items in Nigeria.

Community Support Services (CSS)/ Community Home Based Care Activities (CHBC)

These include;

  • Nursing triage - done at the facility level to assess and refer clients into CHBC services to continue care
  • Supervisory Staff/HBC focal person/coordinators provide ongoing holistic care to patients / their families from the hospital to their homes
  • Team work with community volunteers and other thematic areas for continuity of care
  • Tracking of clients that miss their appointments to ensure retention in care
  • Conduct home visits to those in need of nursing/medical services and adherence support
  • Supply of HBC kits and education of family members on how to use the items for quality care
  • Sensitize and mobilize communities on HIV & TB prevention and stigma reduction
  • Mobilize communities for HIV Counseling and Testing (HCT) and other available services
  • Conduct community activities as organized by volunteers

There are two models used in delivery of CSS/CHBC services. They are the facility based model and the community based model. In the facility based model, a staff from the health facility, volunteers and community members are involved and trained to carry out CHBC services. In the community based model, a resident community based organization, faith based organization or non-governmental organization in a community is identified and assessed to provide CSS/CHBC services through its staff.

Support Group Activities

These involve creating and coordinating all activities of PLHIV support groups. Train peer educators for Interpersonal Communication (IPC) counseling

  • Build capacity of support group members to achieve self-sustainability
  • Link PLHIV within the community for HBC,OVC, IGA, other PLHIVgroups e.g. networks for wrap around activities .
